Mobile Occupational Therapy
What is a Mobile Therapy Practice?

Mobile therapy is outpatient therapy that is performed in the patient’s home rather than at an outpatient therapy clinic. You do not have to be ‘homebound’.  We can continue with therapy after you have been discharged from Home Health.  Mobile therapy is for anyone that may have a limitation or difficulty attending in clinic treatment sessions.  Home based therapy is convenient and functional as it takes place in a natural and realistic environment.

Activities of daily living is a term to describe the self-care, leisure and productive activities an individual completes on a day-to-day basis. This can include community living skills such as budgeting, domestic and personal care activities, leisure activities and involvement in work or volunteer activities.   We work on optimizing functional independence and improving overall functioning.

 Skills2Care® 

Tartan Therapy Services provides the Skills2Care® program to all of its qualified clients. Skills2Care® is an evidence-based program developed by researchers at Thomas Jefferson University and is designed to help caregivers manage the day-to-day challenges more effectively. 

 

The occupational therapist certified in Skills2Care® can help:

  • Manage challenging behaviors such as:

    • Wandering 

    • Pacing

    • Refusing help or care (ex: resisting bathing, refusing help during toileting, etc.)

    • Agitation or "sundowning"

    • Anxiety

    • Repeating questions or phrases

    • Hallucinations/delusions

    • Distressing beliefs/paranoia (ex: believing someone stole something or that someone is in the home)

    • Aggression (verbal and/or physical)

    • Incontinence 

    • Rummaging or hoarding

    • Sexually or socially inappropriate behaviors

  • Make the home safer

  • Reduce caregiver stress

  • Increase meaningful activity engagement 

When integrated into the patient's (individual with dementia) occupational therapy plan of care, Skills2Care® can be covered under the patient's Medicare Part B (outpatient) benefits.

Skills2Care FAQs

Can Skills2Care® be provided to a formal (paid) caregiver?

Yes, as long as that caregiver has worked with the patient long enough to answer questions and can be a consistent participant in the program.

 

How long are Skills2Care® sessions?

Typically, Skills2Care® sessions last for about 1 – 1.5 hours, but this is flexible depending on the patient and caregiver.

How often are Skills2Care® sessions provided?

Typically, Skills2Care® is provided about once per week to allow the caregiver enough time to implement strategies. Again, this is flexible depending on the patient and caregiver.

 

Is Skills2Care® covered by insurance?

Yes, Skills2Care is integrated into the patient's (person with dementia) occupational therapy plan of care. It is reimbursed through Medicare Part B (outpatient) insurance benefits. We may be able to bill as an out of network provider for other insurance plans. Please contact us for additional information. 

 

In addition to Skills2Care® for the caregiver, Tartan Therapy Services integrates home safety assessments and cognitive assessments into the patient’s plan of care for optimal implementation of the program. We are proud to use the Allen’s Diagnostic Module for cognitive assessment, and we are happy to provide our report to the physician if interested. 

 

Process

If you are interested in receiving occupational therapy, we will collect preliminary information regarding your needs, relevant medical history, as well as insurance information.

 

If you would like to receive services, we will schedule an appointment for an Occupational Therapy evaluation at your home to establish a Plan of Care.

 

Plan of care – This is a customized client-centered plan. We work in partnership with clients and their carer(s) to establish short and long term goals to enable people to achieve health, well-being and life satisfaction through participation in daily activities.  We will look at the relationships between the activities you do every day – your occupations – alongside the challenges you face and your environment.

We will create a plan of goals and adjustments targeted at achieving a specific set of activities; plan will be practical, realistic and personal to you as an individual.  The Plan of Care is then sent to your medical provider for approval and signature.

Finances:

  • Approved provider for Medicare Part B

  • For private insurances, we can give you an itemized receipt that you can use to request reimbursement by your insurance company.

  • Private Pay

 

Payment and Reimbursement for Skills2Care

  • Skills2Care® can be provided to caregivers as a part of a patient's treatment and may be reimbursed by Medicare Part A or Part B.

  • Skills2Care® can be provided to caregivers as a consultation service paid for privately
